Alloimmune Thrombocytopenia Treated by High-dose Intravenous Immunoglobulin in Acute Myeloblastic Leukemia

Abstract

|
Although prophylactic platelet transfusions prevent bleeding in thrombocytopenia, they
also stimulate alloimmunization and result in refractoriness to further platelet
transfusions. Patients with refractoriness due to alloimmunization often respond to
platelets obtained from donors matched by HLA typing. The effect of intravenous IgG
on platelet elevation had already been known in 1960's by Schulman, and thereafter the
beneficial effect of intravenous high-dose immunglobulin (IgG) in various immune
cytopenias has been firmly established. Here we report a case of alloimmune
thrombocytopenia, complicated in the course of induction chemotherapy for acute
myeloblastic leukemia, successfully treated by high-dose intravenous immunoglobulin.
